In tandem with the Administration's support and collaboration, Kidney Care FIRST is slated to achieve improvements in kidney care nationwide by focusing on five major pillars: awareness and prevention; patient empowerment; quality and access to quality care; innovation; and, research. In that scheme, some facilities and clinicians chosen at random would receive higher or lower payments based on the rate at which they increase the use of home dialysis and transplants. Early intervention often leads patients who need dialysis to choose the best treatment option for their lifestyle and reduces hospitalizations.

Other initiatives will require new regulations, expected to be proposed later this year. The regulation should expand the definition of allowable costs that can be reimbursed under the Reimbursement of Travel and Subsistence Expenses Incurred Toward Living Organ Donation program, raise the limit on the income of donors eligible for reimbursement under the program, allow reimbursement for lost-wage expenses, and provide for reimbursement of child-care and elder-care expenses. DCI started DCI Donor Services to save and enhance lives through organ and tissue donation and transplantation. Clearer ways to measure how well the nation's 58 organ procurement organizations collect donations from deceased donors. Some do a better job than others, but today's performance standards are self-reported, varying around the country and making it hard for government regulators or the OPOs themselves to take steps to improve.
